Job description

We are looking for compassionate and reliable Disability Support Workers to join our team supporting two male NDIS participants living in Glenmore Park. The participants require moderate daily support at home and in the community and have diagnoses including dementia, schizophrenia and traumatic brain injury (TBI). The role involves providing person-centred support, promoting independence, assisting with community participation and responding professionally to behaviours of concern. Training and ongoing support will be provided.

Tasks and responsibilities

  • Support participants with daily living activities within their home.

  • Assist with community access, appointments and social participation.

  • Administer or assist with medication, including insulin administration where required.

  • Follow Behaviour Support Plans (BSPs) and use positive behaviour support strategies.

  • Respond safely and appropriately to behaviours of concern, including verbal aggression or absconding risks.

  • Maintain accurate progress notes and incident reporting.

  • Transport participants safely to appointments and community activities.

  • Work collaboratively with families, allied health professionals and the wider support team.

  • Promote participant choice, independence and dignity in all aspects of support.
