Job description
We are seeking an experienced and dependable Disability Support Worker to support a client transitioning back into the community after a lengthy period of incarceration.
This role ideally suits someone with previous experience supporting forensic clients who understands the complexities of community reintegration. You will provide structured, person-centred support while maintaining clear professional boundaries and helping the client build stability, independence and positive community connections.
Experience in mental health, psychosocial disability, justice, corrections or community reintegration will be highly regarded.
Tasks and responsibilities
Your responsibilities may include:
Supporting the client to safely transition back into the community
Building a consistent and professional therapeutic relationship
Assisting with daily routines, appointments and community access
Supporting the development of independent living and social skills
Following established behaviour support and risk management strategies
Recognising and responding appropriately to changes in behaviour or risk
Completing clear, accurate and timely case notes and incident documentation
Communicating effectively with the broader support team
