Job description

Axis Supports is seeking qualified and dedicated Support Workers to join our growing team across the Northern Beaches and Greater Sydney. As we continue to expand our services within both the NDIS and Aged Care sectors, we are looking for compassionate and reliable individuals who are committed to making a genuine difference in the lives of the people they support.

This role involves providing person-centred support to help participants maintain their independence, achieve their goals, and engage meaningfully in their communities. We are looking for workers who are dependable, motivated, and committed to delivering consistent, high-quality care and support.

Tasks and responsibilities

Depending on participant needs, responsibilities may include:

  • Providing personal care support including showering, dressing, grooming, hygiene assistance, and toileting

  • Assisting with daily living activities and household tasks

  • Supporting participants to attend appointments and community activities

  • Encouraging social engagement and community participation based on participant goals and interests

  • Providing emotional support and promoting participant independence

  • Supporting participants with mental health challenges and implementing positive support strategies where required

  • Assisting with mobility and manual handling tasks, including the use of hoists and other equipment where appropriate

  • Medication assistance in line with training and organisational policies

  • Maintaining accurate progress notes and documentation

  • Working collaboratively with families, carers, allied health professionals, and the wider support team

  • Following individual support plans and promoting participant choice and dignity at all times
