Job description
We are seeking compassionate and reliable Disability Support Workers to join a Supported Independent Living (SIL) home in Cameron Park, supporting three male participants aged 44, 57 and 66 with diverse support needs including acquired brain injury (ABI), Down syndrome, non-progressive dementia, and organic personality disorder. This is a high-support environment requiring workers who can provide person-centred care, promote independence, and support participants to live fulfilling lives both at home and in the community.
Tasks and responsibilities
Provide personal care support including showering, dressing, toileting, grooming and hygiene assistance.
Assist participants with meal preparation, mealtime support and following mealtime management plans.
Support mobility needs, transfers and manual handling using appropriate equipment and techniques.
Administer or assist with medications in accordance with training and organisational procedures.
Implement Behaviour Support Plans (BSPs) and respond appropriately to behaviours of concern.
Support participants to engage in social, recreational and community activities based on their individual interests and goals.
Assist with household tasks including cleaning, laundry and maintaining a safe living environment.
Transport participants to appointments, activities and community outings.
Complete accurate documentation, progress notes and incident reporting.
Work collaboratively with families, allied health professionals and the wider stakeholder team to achieve positive outcomes for participants.
