Experience supporting people living with schizophrenia, paranoia or similar mental health presentations is highly regarded.
Experience supporting hospital-to-home transitions.
Strong understanding of trauma-informed practice and de-escalation techniques.
Ability to work consistently with one participant and build trust over time.
Comfortable collaborating with clinical teams and following behaviour support plans.
Job description
We are seeking an experienced male Disability Support Worker, based on participant preferences, to provide one-on-one support to an NDIS participant living in the Richmond area following a recent hospital discharge. This is a rewarding opportunity to make a genuine difference by supporting someone with complex psychosocial needs to transition safely back into the community.
The participant experiences significant mental health challenges, including paranoia, and requires a calm, reliable and trauma-informed support worker who understands the importance of building trust over time. To support continuity of care, we are looking for one consistent worker rather than a rotating team.
Tasks and responsibilities
Provide individualised support in the participant's home and community.
Assist with the participant's transition from hospital back into everyday life.
Build a strong, trusting and professional relationship through consistent support.
Follow the participant's behaviour support plan and clinical recommendations.
Work collaboratively with the participant's treating clinicians and support network.
Use trauma-informed and de-escalation approaches to support periods of distress.
Support community access and meaningful activities aligned with the participant's goals.
Maintain accurate progress notes and communicate professionally with the wider support team.
Promote the participant's independence, dignity and wellbeing at all times.
