Closer Care is looking for experienced, caring and reliable Disability and Mental Health Support Workers to join our team in Sydney’s Eastern Suburbs.
This is a role for someone who enjoys helping people feel more confident, independent and connected to their community. You will support participants in their homes and out in the community, based on what matters to them and the goals they want to work towards.
No two shifts will look exactly the same. You might help someone get ready for the day, prepare a meal, attend an appointment, go shopping, enjoy a community activity or work on everyday skills that build independence.
At the heart of the role is building trust, listening well and providing support with patience, respect and kindness.
Tasks and responsibilities
Depending on the participant and their support needs, your shifts may include:
Helping with daily routines such as cooking, cleaning and laundry.
Supporting with personal care, including showering, toileting, hygiene and getting ready for the day.
Providing prompts and encouragement with everyday tasks.
Supporting participants to attend appointments, go shopping or take part in social and community activities.
Helping participants work towards personal goals such as budgeting, communication and building confidence.
Supporting people living with psychosocial disability, mental health concerns or challenging behaviours.
Transporting participants in your own registered and insured vehicle.
Completing clear progress notes after each shift.
Keeping in regular contact with Closer Care leaders, support coordinators and other team members.
Following support plans while always respecting each participant’s choices, preferences and independence.
