Job description
We are looking for caring, reliable and resilient Disability Support Workers to support three adult NDIS participants living together in Emerton. The participants have a range of disabilities including Down syndrome, intellectual disability, cognitive delay and developmental delay. This is a high-support environment where you will assist with daily living, community access and implementing behaviour support strategies while helping participants live as independently as possible. Training and ongoing support will be provided.
Tasks and responsibilities
Provide personal care including assistance with hygiene, showering, dressing and toileting as required.
Support participants with daily household tasks and meal preparation.
Assist participants to access the community, attend appointments and participate in activities they enjoy.
Provide safe transport to appointments and community outings.
Follow Behaviour Support Plans (BSPs) and respond appropriately to behaviours of concern.
Support participants with mental health needs using a calm, person-centred approach.
Monitor participant wellbeing and report changes or incidents accurately.
Complete daily progress notes and required documentation.
Work collaboratively with other support workers and the wider care team.
Promote participant choice, dignity and independence in all aspects of support.
