Job description
We are seeking a compassionate and reliable Disability Support Worker to provide in-home support for a participant living in Kellyville who has experienced a stroke. This role involves assisting with personal care, mobility support, community access while promoting independence, dignity, and quality of life.
The participant requires support with daily living activities, mobility and wheelchair-accessible van. This role would suit an experienced support worker who is confident providing personal care and supporting participants with complex physical support needs.

Tasks and responsibilities
Provide personal care including showering, dressing, grooming, hygiene support, and toileting assistance.
Assist with transfers and mobility.
Support the participant with daily living activities in the home.
Assist with community access, appointments, shopping, and social activities using the wheelchair-accessible van provided
Encourage independence and participation in activities chosen by the participant.
Monitor wellbeing and report any concerns or changes.
Maintain accurate documentation and progress notes.
Follow individual support plans and organisational policies and procedures.
